Position changes comfort, duration, and how much either partner has to hold themselves up — separately from technique, which our oral sex guide already covers well. Five real setups below, from lying back to fully mutual.

One-way positions

1. Lying back, edge of the bed

The receiving partner lies back with hips at the edge of the bed, feet flat on the floor or legs draped over the giving partner's shoulders. The giving partner kneels or sits on the floor in front — this is the lowest-effort setup on the list, since the receiving partner is fully supported and the giving partner can adjust height by sitting back on their heels or leaning in.

2. Seated on a counter or chair

The receiving partner sits on a counter, chair, or bed edge at a height that puts the giving partner's head roughly level while kneeling. Raising the receiving partner rather than lowering the giving partner tends to be easier on the knees over a longer session, and it gives the receiving partner a genuinely stable, hands-free base.

3. Standing

One partner stands while the other kneels in front, head at hip height, hands free to guide pace and pressure. It's the most search-common named version of this and for good reason — the standing partner's hands stay free the whole time, and there's no furniture required beyond floor space. A wall behind the standing partner adds stability for longer sessions.

Mutual positions

4. 69

Both partners give oral to each other at the same time, in a mirrored, inverted position — most commonly one partner on top facing the other's hips. Simultaneous stimulation appeals to some people and splits focus in a way that's more distracting than additive for others; neither reaction means you're doing it wrong. Full entry in the glossary.

5. 69, side-lying

The same mirrored setup as above, but both partners on their sides rather than one on top of the other. This removes the weight-support demand of the on-top version entirely — worth trying specifically if holding position, not the concept of mutual oral itself, was the actual obstacle before.

On comfort over a longer session: whichever position is doing the "holding," check in on neck and jaw position specifically — those tire faster than most people expect and rarely announce it until it's already uncomfortable.
